How to Purchase a Disclosure Ticket
- You can purchase a “disclosure ticket” using a multimedia station at 7-Eleven/FamilyMart storefront or Loppi at Lawson/Ministop storefront. (Booking code is the same for all convenience stores.) When purchasing a disclosure ticket, please purchase one worth the amount of the desired delivery option in addition to the disclosure fee.
Please click here to see how to operate the machine. - One disclosure ticket is required per name. When requesting disclosure for two or more names, such as your current family name and maiden name, please purchase additional ticket(s) for the additional name(s).

- Disclosure ticket is valid for 6 months after the date of issue.
- We will not issue any receipt for the purchase of disclosure tickets.
- Handling Charges are non-refundable.
- Some convenience stores may not have a multimedia station / Loppi.
- If you have already purchased a disclosure ticket for "Personal delivery (special type)"(JPY1,650) and wish to add "Express Mail" later, please purchase a fixed-amount postal money order for "Express Mail fee" at Japan Post Bank.
